Health Equity Awards Nominations

2022 Heartland Regional Conference Health Equity Awards Nomination Form

The Heartland Conference on Health Equity and Patient Centered Care awarded our first Healthy Equity Award in 2018 recognizing individuals who demonstrate a commitment to health equity by reducing disparities in Region 7, including Iowa, Kansas, Missouri & Nebraska. Awardees have shown a dedication to health equity and reducing disparities in historically disenfranchised communities including racial and ethnic minorities, immigrants and refugees, individuals with disabilities, sexual and gender minorities, and those living in rural areas. The Heartland Conference on Health Equity and Patient Centered Care will also award a Regional Community Health Worker Award. This award will recognize a Community Health Worker dedicated to health equity.

Please read the awards criteria and complete one nomination form per nominee for the Health Equity award or the Regional Community Health Worker award, to be awarded on Friday, September 9th, 2022, at the 2022 Heartland Conference on Health Equity and Patient Centered Care.

WHO IS ELIGIBLE?

NUMBER OF AWARDS

Individuals who demonstrate equitable work in health and areas which support health. Individuals must live or work in HHS Region 7 (Region 7 includes Iowa, Kansas, Missouri & Nebraska).
Health Equity Awardees: Up to four individuals will be awarded each year.
Community Health Worker Awardees: One community health worker will be awarded each year.

AWARD CRITERIA

The awards, including the Community Health Worker Award, will go to an individual actively working, or who have dedicated their career to advancing health equity. Awardees will work, or have worked, with historically disenfranchised communities including racial and ethnic minorities, immigrants and refugees, individuals with disabilities, sexual and gender minorities, and those living in rural areas.
Awardees may work in areas of improvement which may be related to Centers for Medicare & Medicaid Services Meaningful Measures priorities, including:

  1. Promoting Effective Communication & Coordination of Care
  2. Promote Effective Prevention and Treatment of Chronic Disease
  3. Work with Communities to Promote Best Practices of Healthy Living
  4. Make Care Affordable
  5. Make Care Safer by Reducing Harm Caused in the Delivery of Care
  6. Strengthen Person and Family Engagement as Partners in Their Care.

SELECTION PROCESS

The 2022 Heartland Conference on Health Equity and Patient Centered Care planning committee will review all nominations and vet candidates for consideration and approval.

AWARD

Awardees will be awarded on Friday, September 9th, 2022, at the 2022 Heartland Conference on Health Equity and Patient Centered Care. The awardees will receive regional recognition for their work through various communication channels.
